Driving Directions
5th Ave Professional is a four story light blue building. It is located on the East side of 5th Avenue directly across from the Northgate Mall. The clinic is located on the 4th floor which can be accessed by elevator.
Parking is available on the South side of the building. The main stairs lead to the elevator. From the entrance, please follow the signs to suite 406.

Head north on I-5.
Take exit 173 to Northgate Way.
Turn left on 1st Avenue.
Turn right on Northgate Way.
Turn right on 5th Avenue.
Take second left into the 5th Avenue Professional building.

Head south on 1-5.
